Applying for Residency

Residential Aged Care:
The admissions process is two-fold:

1. Aged Care Assessment Team (ACAT) Assessment
All prospective residents require governmental approval before entering any aged care facility.  Prospective residents should make an appointment with their local ACAT to independently assess their suitability for residential care.  This assessment is a free service available from most Area Health Services; however a General Practitioner or Hospital referral will be required.  Our Social Worker will be able to advise on how to organise the ACAT Assessment.

2. MZH Assessment and Admissions Process
The admissions process is completely confidential.  Contact reception on (08) 9375 4600 to request an Application Pack.  Once completed and returned, the prospective resident and interested family members will be invited to meet the Social Worker to discuss the process and answer any questions or concerns. 
There may be a waiting period before a place becomes available.  Placements are prioritised on the basis of individual need.  The MZH has a waiting list for prospective residents who may wish to enter the Home at a later stage, or in the event that a bed is not immediately available.
Once a place has been offered by the MZH, potential residents must meet with the Finance Administration Manager
